There are three core modules within the HEALTHeWV application:
HEALTHeSURVEYS: Allow for information collection directly from a patient, making that information available immediately to the provider during the same visit. Prior to seeing the health care provider, the patient completes a variety of automated surveys; the surveys are then administered via workstation-based Web surveys or handheld electronic data capture devices.
HEALTHeCARDS: A “just in time,” scorecard-type of tool that compares specific clinical test results and provider input to pertinent clinical practice guidelines (CPG) and then generates a health status report based on CPG compliance. Those HEALTHeCARDS currently used in the program are Diabetes Mellitus, Women’s Health, Wellness, and Cardiovascular Risk Reduction. Additional HEALTHeCARDS for other chronic disease conditions, such as kidney disease, will be developed in the near future.
HEALTHeNOTES: HEALTHeNOTES is the outpatient medical record documentation tool that incorporates data from a variety of sources (e.g., eSURVEYS, eCARDS, lab/radiology results, notes from previous visits, prescription information, and master problem list) so that providers can quickly and easily assess identified issues and document an encounter in accordance with easily referenced clinical practice guidelines, medical record documentation guidelines and Joint Commission on Accreditation of Healthcare Organizations (JCAHO) required data collection elements.
